Adult Life Planning (ALP)

Services  focused on educating and supporting the family in accessing adult community programming on behalf of the transitioning children and young adults. This service will emphasize the development of a treatment plan based on a person-centered assessment of the individuals current functioning and needs  in support of decision making, planning for adulthood and continuity of services for successful transition outcomes.

Family Consultation (FC)

Family consultation provides education, training, guidance, collaborative planning with community and state service agencies to enhance the families understanding of local and state services that  supports an individual’s transition to adulthood. Consultation focuses on reinforcing strategies that promote independence, consistency across environments, and long-term planning aligned with the individual’s preferences and goals.

Intensive Individual Support Services (IISS)

One-on-one support is provided to the child or young adult through the implementation of an individualized plan with goals identified to target daily living skills, social interactions, and community safety.. IISS  emphasizes consistency with positive behavior support across the home and community environments by providing opportunities and modeling alternative ways of behaving for the child and young adult.

 

Respite Care

Short-term supervision and assistance with activities of daily living that are provided to individuals because of the absence or need for relief of those persons normally providing care.  Respite Care services provide relief to the primary caregiver within the home or an approved state Respite Care facility. The services are designed to ensure the continuity of services by maintaining established routines and behavioral strategies in the home or community setting.
